What we may receive
Information may include contact details, relationship to the participant, preferred communication, broad disability support needs, service timing and funding status. A professional referral may contain additional information submitted through AMA’s embedded Fillout form.
How it is used
AMA uses the information to respond, check service fit and availability, arrange a conversation, coordinate an authorised referral and meet applicable service, safety and record-keeping responsibilities.
Consent and authority
Participants can decide who is involved in communication. Before sharing personal information with family members or professionals, AMA may need to confirm participant consent, nominee arrangements, guardianship or another lawful authority.
